Everyone has goals, dreams, and aspirations in life, and it is the job of Cornerstone, together with you, to ensure that these are as far as possible turned into reality.

The Individuals we support are supported in a person-centred way and are actively encouraged to have control over their own lives and decision-making processes.

Cornerstone helps you to facilitate and achieve your hopes, dreams, and aspirations; all of which leads to a good and better quality of life all this will be achieved in an open, honest, transparent, approachable, and supportive way.

I have a wealth of skills and experience including supporting individuals who have a diagnosis of autistic spectrum disorder, learning disabilities and mental health conditions.

We will always work hard to give you the support you need to get on with your life and help you to live your life in the way that you want to.

We can help with all aspects of daily living, including personal care, finances, cooking, cleaning, and safety at home. Our outreach service helps you to access education, leisure and community facilities. We can support you whilst meeting friends, visiting new places or trying new things.

 

All our Support is bespoke to you. 

Support Planning

We will support the individual to co-produce a plan focusing on person centred outcomes in line with their budget.

 

It is important to remember your Plan can be written how you want it to be. Your Support Planner will have lots of experience in putting Support Plans together and can give you lots of advice.

Your Support Plan will contain lots of great person-centred information about you and will also contain the important information needed including all the cost of the support you have chosen.

MATCHDAY BUDDIES

Matchday Buddies is a local Not for Profit project based in the Merseyside, Wirral and Cheshire West and Chester which provides a unique service enabling children (8 years and over) and adults with disabilities and people who are disadvantaged to access their favourite Football, Rugby even Basketball club or any Sporting event with assistance from a dedicated team of qualified and fully trained Volunteer staff. 

 

People with disabilities and people who are disadvantaged are less likely to attend a  sporting event of their choice than people living without a disability / disadvantaged. Why? It’s simple; there is not enough support to assist them in attending the games, financing the tickets, and ensuring they are safe when there.

Matchday firmly believe that children and adults with disabilities and who are disadvantaged here in the UK should have the same rights, freedoms, responsibilities and quality of life as those without. As a result, Matchday Buddies, a small NFP projet with big ambitions, is due to launch officially in Early  2024. 

Companionship

If you receive a Personal Budget, Personal Health Budget, or fund your own care and support we can work with you to develop a package of support that is built around your own needs and wishes.

 Our Companionship Service is a one to one based service for clients. It contains a full range of companionship from simple conversation and reminiscence to trips out and much more

Help from Cornerstone to create bespoke adverts, job descriptions and person specifications

Give out our email address to people returning completed application forms

Have support with shortlisting candidates

Support to design interview questions

Support to arrange a venue for the interviews

Help to collect references and creation of an employment contract

SUPPORT PLANNING

I help you develop your personal support plan based on the amount of budget you have available. I will work with you to ensure that you use your budget flexibly so that you are supported where you want, when you want, how you want it and with whom to ensure that it meets your needs and aspirations. 
I simply charge you per hour of advise and assistance, so you can decide how much or little of our time you want or need

COMPANIONSHIP

I understand the cost of companionship and care can be hard to determine.

That's why I make our pricing transparent, so you can make an informed decision.

PA RECRUITMENT 

I can offer assistance to individuals in this area who are in receipt of direct payment /Self Funder and wish to employ a Personal Assistant / Assistants,

I can help all areas of the process to advertise and recruit a Personal Assistant who is best suited to you.

We can be involved as much or as little as you would like us to be.
Costs are subject to the involvement required.

My Name is Wayne I have worked in Adult Health and Social care for 30 years with hands on experience in a variety of services. Job roles included supporting Service Users in their day to day lives, liaising with families, Health Professionals, and outside agencies. I have supported managers and been a manager myself and supported teams in identifying issues, planning improvements, and implementing them.

 

I have worked across all service sectors from Care Leavers, Learning disabilities, Autism to the Elderly. I have worked and Managed in Elderly Care Projects, supported living and Respite for young Adults with Learning difficulties and Autism and Managed a Day Service for Adults with support needs.

     

I have qualifications to an NVQ requirement and recently undertook my Safeguarding Level 5 qualification, I’m an experienced Person-Centred Planner / Support Planner and have experience with all types of planning including Health Action Planning, Support Planning and Pathway Planning. I have supported young Adults with Transition Planning from School into the Adult services / into the community.

 

I have recruited Personal Assistant teams for individuals both Elderly and those individuals who have Learning Disabilities and Autism who receive Direct payments along with Self funders I have put into place Adverts, contracts, reviews, Insurance, emergency cover, interviews, and references. 

 

Outside of work I’m a keen sporting enthusiast, dabble in Graphic Design and like to Cook. 

 

The areas I can cover are Little Sutton, Great Sutton, Ellesmere Port and The Wirral.
