Cornerstone Support Services is a Micro Provider registered with ‘Small Good Things’ and been through the ‘Community Catalstys Programme’ and I’m also registered with the Business Directory on the Cheshire West and Chest website.

Cornerstone is a high-quality provider of person-centred social care services for people with learning disabilities, mental health and other needs.

To support our service users we provide a range of services to meet their differing needs, including Housing Support, Outreach and Companionship also including Support Planning and PA Recruitment.

I work with people who have a wide spectrum of needs, ranging from those who require short term support to regain their independence to people with complex needs who require intensive support to enjoy a good quality of life.


I have a background in Transition planning and support young people as they plan to leave school and plan their own futures through Person Centred Practices.  

Housing Support deliver this service by providing practical support in people’s homes. I can assist people with things like:

  • Support with paying bills

  • Menu planning, cooking, and shopping

  • Help with domestic tasks/cleaning.

  • Support to attend and make doctor’s appointments & other health appointments.

  • Help to arrange repairs.

  • Help with correspondence.


My services are individually tailored. I provide housing support that meets people's specific needs which we will document this through a support plan.

Support is put into place to achieve whatever the person requires to enable them to live in their own homes as independently as possible.

I encourage each person to achieve their goals and tailor support to meet individual needs, promote enablement and deliver outcomes

Outreach support people to do specific things, such as help with going shopping, attending college, using public transport or managing money. This might be for a few hours a week, or much more. Often, people who receive outreach support live on their own, or with families or partners.

There can be different levels of support and for varying periods. We can work with the person, their family, advocates, Social Workers and health professionals to ensure support is aimed at enhancing skills and community networks in a safe and enabling way giving the skills and support to live independently and achieve full potential.


Support Planning : I am a Person-Centered Planner and specialise in Person Centered planning / Support Plans/Transitional Planning your support plan is your opportunity to explain what care and support you feel would meet your needs in the best way. I can provide the service User with information and advice via telephone or email, about how to complete their support plan. I can review their support plan before they submit it to the local social care team for approval.

The Support Plan is written in a way that makes sense to the service user and reflects their individual preferences and they are consulted all the way throughout. The planning is kept as cost effective as possible to ensure the person has as much of their budget available to them to meet their support needs.

The first session with a service user is always free no matter how long they need to go through the process, ask questions and understand what will happen, service users are also reminded at that point that they are under no obligation to continue with us at this point.

PA Recruitment: I will help you employ a Personal Assistant (PA) and prepare you to become an employer. I can help to ensure all the relevant paperwork is in place to protect you as an employer including Risk assessment and Job Descriptions, advertisements, Interviewing and recruiting a PA, CRB Checks and Reference checking, A written contract of Employment – Showing holiday entitlement, Training requirements. This service is free to anyone who uses our Support planning service.

Case Management also offers a full Case Management Service for full Management Support to teams of Personal Assistants. We are employed by the Employer (person in receipt of Direct Payment to employ PA’s) to act on their behalf to Manage their PA’s. Included is the Recruitment Service, Employment Service as well as training to Personal Assistance, monthly reviews, supervision to PA’s, dealing with all annual leave, rotas, timesheets and payroll. We deal with all disciplinary and grievance procedures and provide spot checks,  We also provide anything necessary such as MAR sheets, communication logs, Epilepsy Diaries and much more together with a Safeguarding Role. This service is available where there are 2 or more Personal Assistants employed.
